Most of the people inside a hospital cannot leave it of their own accord. An inpatient cannot be moved, a sleeping patient should not be woken, and a device in intensive care will not forgive even a brief voltage dip. Those three conditions settle in advance where technical work in the building can happen, when, and at what noise level. In clinics and private health facilities the scale drops; the conditions themselves do not change.

Surfaces are wiped with disinfectant many times a day, so a wall-mounted reader, button or loudspeaker is chosen from material that will survive that cleaning. Along operating theatre and laboratory corridors, any drilling, cutting or dust-producing operation is separated by a temporary barrier from the section where patient flow continues. A refurbishment can close one wing of the building but not all of it, so cabling is divided to be taken over wing by wing. At night the audible warning level cannot match the daytime one, and paging and call bells are zoned so they do not spill into patient rooms.

In a laboratory and in a blood storage cabinet, temperature matters as much as the product, because a sample once taken cannot be taken again. When a cabinet door is left open, the person who would notice is usually on another floor. At a point like that it is not enough for the warning to appear on a screen. The alert keeps going until it reaches someone who is in the building at that hour and able to act, and who saw it, and when, goes on record too.
